האם אנחנו לומדים מהניסיון לטובת מניעת תאונות?

האם זה מקרה, שבאותה החברה, באותו דגם, אירעו שתי תאונות עם מאפיינים דומים? שתי תאונות בהן הייתה גישה לא מיוצבת שחייבה הליכה סביב, נגיעה עמוקה, התעלמות מקריאות להליכה סביב של הקצין הראשון ולבסוף ניסיון להכנסת רברסרים אחרי הוצאתם, בניגוד לספרות? למרבה הצער, התשובה לכותרת, במרבית המקרים, שלילית.
